Hesitant but coaxed on by his wife Lady Macbeth who holds his right hand holding the knife and points at the sleeping King Duncan with her right hand to kill him, Macbeth's face portrays both fear and bloody passion for the heinous crime that he is about to commit.
On his other side are the three evil spirits who prophecied his ascendency to the throne. The first one offering him with the murder weapon while the second one is pointing to the sleeping King and provoking Macbeth to kill him, while the third one celebrates the immediate triumph of fate for Macbeth that is about to follow after he has committed the violent crime.
King Duncan on the other hand is fast asleep while Macbeth slowly removes the cover over his body so that he can pierce him with the knife thus highlighting the cold bloodedness of the murder.
Macbeth himself is alone in the King's bed chamber as described in the play, with the words of his wife and the prophecy of the three evil spirits playing in the back of his mind as he is tormented between his ambition and greed and his inner conscience.
The imaginary presence of Lady Macbeth and the three evil spirits in the painting merely symbolizes Macbeth's great mental torment as he struggles to choose the path that his wife and the three evil spirits want him to follow and the King who trusted and loved him so deeply that he took his invitation to be his guest for the night without any hesitation.
